2. Telephone number and hours in which contact person can be reached if necessary. |

4. Type and version of operating system |
|
5. Type and version of web browser |
|
6. Detailed explanation of the problem including exactly where and when the problem
occurs, whether the problem occurs differently each time or is consistent, and the exact
error message if there is one. |

7. The steps which you have already attempted to solve the problem. |
